To begin your ADHD assessment, please complete the referral form below. This allows our clinical team to gather the information needed to determine whether the service is appropriate for you.

Once you have submitted your referral form, you will be guided to the payment page to confirm your assessment booking. Following payment, your pre-assessment stage will begin. This includes completing structured pre-assessment questionnaires and a clinical triage review to ensure a safe and appropriate assessment pathway.
If appropriate, you will then be invited to proceed to the comprehensive assessment stage with one of our qualified ADHD clinicians.

Please note: If our clinical team determines that the service is not appropriate for you, you will receive a full refund.
